The purpose of this survey is to gain insight into how well the Agency is meeting your and your family’s needs.  It should only take you a couple minutes

to complete it. The Agency uses the responses that we get, including yours, to improve the interventions and services that we offer. 

 

Your responses are 100% confidential.  You cannot be identified by your responses and individual responses are not shared.  Your responses will be mixed with all the other responses that we receive. 

 

The first section of the survey gathers basic information about your family such as: your age, the number of children you have, the source of your household income, where you live by Zip Code, etc.  This information is collected to help the Agency better understand the makeup of the families we serve.  Again, you cannot be identified by your responses and individual responses are not shared.
